Facility Fees
- This facility's fees cover the use of the facility only. The facility fees do not include laboratory, pathology, surgeon, anesthesiologist, certified nurse anesthetist fees, nor does it include the cost of any implants used for your surgery. You will be billed separately for these fees.

Types of Bills
Here is a brief explanation of the possible charges which you may receive following your procedure:
> Surgery Center Fee
This fee is for the staff and equipment provided for your safe and successful experience there at Tri City Surgery Center. Questions and payments regarding your surgery center billing should be addressed at the surgery center.
> Physician Fee
Your procedure will be performed by your surgeon. Since this physician is not an employee of our surgical center, he/she will bill you separately for your procedure,. The physician's bill, along with any questions pertaining to it must be addressed to their office.
> Anesthesia: Arizona Anesthesia Solutions
If you receive anesthesia from an anesthesiologist or CRNA during your procedure, you will receive a separate bill for these services as well. You must address all questions and send all payments to that respective billing group or physician. http://azanesthesiasolutions.com/insurance/
Other Charges: Inform Diagnostics, Covenant Pathology, Lab Corp, Sonora Quest, YRMC Lab, Breg,Inc., and MedVantage.
Depending on several factors related to your particular procedure, you may receive other charges. These may include Durable Medical Equipment (such as crutches, braces, etc.), lab fees or other services not performed at Tri City Surgery Center. Questions for these must be directed to the respective company or provider.
Collections and Payment Policy
- Be prepared to pay any co-pay or deductible amounts on the day of your surgery.
- Patients who do not have insurance coverage or those having cosmetic surgery are also required to pay their charges in advance.
- We will bill your primary and secondary insurance carriers or governmental agency directly for the facility's charges.
- This facility accepts cash, cashier's checks, credit cards, and personal checks (not to exceed $300) with a valid I.D.

- We will bill your primary and secondary insurance carriers or governmental agency directly for the facility's charges.
- Be sure to bring your most current insurance, Medicare, or public assistance card with you on the day of your surgery. If you have more than one insurance carrier, we will also need accurate secondary billing information.
- Please be aware of any admission policies your insurance plan may have. You or your physician may have to adhere to certain requirements in order to ensure maximum reimbursement. Failure to obtain pre-authorization and physician referral may greatly reduce or eliminate your benefits.
